Liabiliy Waiver: I am aware that there may be an inherent risk for injury while participating in the auditions. Though every safety measure is being practiced, I understand the possibility of these risks and agree to indemnify and hold harmless Dancing Through the Curriculum (DTC). 

I am aware that I am providing my information and photo at my own free will. I am also aware that photos, voice, video images, and/or recordings will be taken during all activities. I am aware that the headshot/photo I submit, as well as photos and videos taken at the event will become the sole property of DTC, and posted on several websites/social media sites of DTC. DTC is not responsible for photos, information, or video images that are taken from our websites/social media page(s) and posted on other public viewings. I give permission for my child's photos and video images to be posted on the above entities website(s) and social media page(s).

I understand that all company member decisions are made final and will not be changed or altered. I accept nomination and will be in attendance at my indicated scheduled audition. I understand there are no refunds of any tickets purchased or donated.
